Pain is a highly complex sensory and emotional experience that many health care professionals are challenged to treat. This may be due in part to a paucity of training during our formal training years, leading to confusion about, disagreement over, and inconsistency within treatment paradigms for treating our patients. In this lecture an attempt will be made to clarify what acute and chronic pain are, how they are the same and how they differ, and how optimal treatment of each should also differ. An overview of the neurophysiology of pain will be given. Risk factors for poor postoperative pain outcomes will be discussed. A primer on pain pharmacology will be discussed. Lastly, the future of pain treatment will be explored. The intent of the lecture is to help health care providers make the treatment of acute and chronic pain more standardized and hopefully more effective for our patient populations.

Learning Objectives

  • Describe the epidemiology and clinical definitions of acute and chronic pain and their impact on patient outcomes

  • Differentiate the neurophysiologic mechanisms underlying acute pain, chronic pain, and pain centralization, and explain how these mechanisms influence treatment selection

  • Evaluate the indications, mechanisms of action, benefits, and limitations of major analgesic drug classes and emerging pain management therapies for the treatment of acute and chronic pain

Course Staff

About the Speaker

Robert Giering, MD

 Dr. Giering is board-certified in both Physical Medicine & Rehabilitation and Pain Management. His fellowship training in Interventional Pain Management was completed at the University of Virginia Medical Center Department of Anesthesiology. Dr. Giering completed undergraduate and master’s degrees majoring in physics and bioengineering at Colgate University and Dartmouth College. He received his M.D. from Albany Medical College in New York. Dr. Giering was also a Chief Resident in PM&R at the University of Virginia Medical Center. Dr. Giering has been on the medical staff in the Department of PM&R and Pain Management at Rutland Regional Medical Center, and Southwestern Vermont Medical Center in Bennington, VT. He is a fellow member of the American Academy of Physical Medicine and Rehabilitation, American Society of Regional Anesthesia, and the North American Spine Society. Dr. Giering has published book chapters and numerous scholarly articles and abstracts and has given frequent lectures at regional and national meetings.
